Why distance makes this harder
A favorable document may not by itself explain finality, jurisdiction, service, implementation requirements, or later proceedings. The family may describe non-implementation before a qualified person has reviewed the complete record.
Critical questions
- Is the document authentic and complete?
- Is it final, current, and directed to the relevant authority?
- Has it been served or communicated through the required process?
- Is an appeal, stay, correction, or related matter pending?
- What response has the office given?
- What qualified review is needed?
- What official references exist?
What a responsible future system would need
- Certified record review
- Procedural-status verification
- Qualified legal or administrative review
- A precise written implementation request
- A record of service, reference, response, and deadline
- A no-public-accusation default
Safeguards
OPCA must not assume misconduct merely because the expected result has not occurred. The complete procedural position must be understood first.
What OPCA is testing in Stage One
- Who verifies finality and authority?
- How is an official response requested and stored?
- When does delay become a pattern suitable for public reporting?
- How are corrections handled if OPCA misunderstood the record?
